You
probably recognize that food cravings are the reason that
you’re overweight. You know that there are certain
foods that you simply cannot quit overeating. Whether it’s sweets,
or chips, or pasta, or bread, or potatoes, or Mexican food—or

very long.
You
literally feel out of control of your eating. You may go on a diet,
and at first you do pretty well. But within a few days you feel increasing
urges to eat your favorite food—not just to eat it, but to overeat
it. Why? What’s going on?
The
answer is that you are addicted to certain foods
just as if you were heavily addicted to cigarettes. Recent medical
research shows that, for those of us with a weight problem, certain
foods create the same chemical dependency in the brain that cocaine
does. When you go too long without your food “fix” those
areas in the brain generate irresistible cravings. You can try to
resist for a while, but eventually everyone gives in and overeats
the forbidden foods.
Those
of us with the strongest predisposition to overweight are probably
born pre-wired to become addicted to food. Research shows that overweight
persons have fewer receptors for the normal brain biochemical dopamine
and this deficiency may make it easier to become addicted to certain
foods.
The

the bright white lines are dopamine receptors. The brain on the left
is from a normal weight person. The brain on the right is from a seriously
overweight person. The overweight person has far fewer dopamine receptors
than the normal weight person. Overweight persons may be overeating,
in part, due to lower levels of dopamine receptors.
As
with any other type of addiction, food cravings are usually worsened
by stress or emotional upset. You’ve probably noticed that when
you’re upset, lonely, bored or angry you want more “comfort
foods”—your favorite carbs, of course. Depression and
anxiety lead to more carb cravings for most of us.
Some
of us have spells of such intense carb cravings that we go on a binge.
By this I mean eating a huge quantity of food in a quick, uncontrolled
manner: a whole package of cookies, a whole cake, a whole bag of chips,
etc. Others of us wake up in the middle of the night with uncontrollable
carb cravings. We find ourselves in the kitchen at 2 a.m. eating our
favorite carbs.
Because
food cravings are ultimately irresistible, simply trying to tough
it out or to use willpower to overcome them is futile. It won’t
work. The only reasonable solution is to use safe mediations that
will stop the food cravings where they start—in the brain.
The
safe, modern medications that we have today typically stop
all cravings for excess food within 48 hours. They stop stress
eating, eating due to depression and anxiety, binge eating, and nighttime
eating, too. And the medications usually keep on working indefinitely.
When
I mention using medications to help lose weight, you may have frightening
thoughts of “diet pills” that cause medical problems or
even addiction. You may remember the problems with “phen-fen”
back in the mid-90s, or the problem with amphetamine diet pills in
the 1950s and 60s. You’re probably aware of the recent ban on
ephedra. You may have had disappointing experiences with Xenical or
Meridia. You may have lost weight with diet pills, only to regain
when the pills were stopped. So naturally, you’re skeptical
about taking medication to help lose weight.
The good news is that we now
have very safe medications to stop food cravings with virtually
no side-effects in the vast majority of patients. My years of experience,
and the many years of experience of my colleagues around the world
attest to the safety of these medications. I keep up with the research
in this area very carefully, and I am not aware of even one medical
paper that has documented significant medical problems with these
medications.
Of
course, there’s no need to lose weight if you’re going
to regain it. I know that overweight isn’t like a sore throat
that we treat and then forget; it’s like arthritis: we get it
under control and then we work lifelong to keep it under good control.

lifelong to help maintain the weight you lose.
Once
we have your food cravings stopped with medications, you can follow
any type of weight-loss diet easily. I’m flexible and will work
with your diet preference; however, the diet that I have seen the
best results with (and the one I follow myself) is the low-carbohydrate
diet. This diet produces the fastest weight loss, with no hunger and
no medical problems.
At
least 4 medical studies in the past few years show that a low-carb
diet results in faster weight loss, more weight loss, and improved
cholesterols and triglycerides. Fears about ketones, osteoporosis
or kidney problems on low-carb diets have been shown to be unwarranted.
Low-carb
diets work best, probably, because they lower your insulin level the
best. Insulin causes body fat storage: the more insulin you make,
the more fat you store. A low-carb diet causes the least increase
in insulin levels.
With
all the new low-carb foods and restaurants today, it’s easy
to follow a low-carb diet for the rest of your life. I’ve been
on my low-carb diet since 1998, and it’s an easy way of life.
Of
course, exercise has a place in weight loss—but perhaps not
what you’d expect. We’ve learned that the best use of
exercise is to lower insulin. Remember, lowering insulin reduces stored
body fat. The type of exercise that lowers insulin the best is strength
training.
By
strength training we mean weight lifting, or using the weight machines
at a gym, or using a BowFlex-type machine, or simply using the elastic
band exercisers you can buy at Wal-Mart.
The
really good news is that strength training takes just 30 minutes,
twice a week. With that schedule you’ll add more muscle protein
for a full 2 years, lowering your insulin and further reducing your
fat storage. Combined with a low-carb nutrition program, strength
training will help you lose the most excess body fat.
Aerobic
exercise (jogging, walking, running, swimming, treadmills, Nordic
Tracks, stair-steppers, dance, etc.) are good for your heart and lungs;
but they don’t do much to help you lose excess body fat (unless
you run several miles per day). So switch to strength training twice
a week for the most effective weight-loss exercise.
My
overweight treatment program starts with a private visit between you

we should be able to start you on the most appropriate medications

After
